The Health Reimbursement Plan is 50% reimbursement for the following copay expenses:
- Outpatient Surgery Copayments – up to 75% per occurrence
- Inpatient Hospital Admission Copayments – up to $350 per occurrence
- High Tech Imaging Copayments – up to $50 per occurrence
After a participant incurs an eligible expense, you should send a claim form along with the details from the insurance company showing the date and description of the eligible copay and you will be reimbursed directly. You have to 30 days after the plan year ends on 6/30 to get any outstanding claims in to be reimbursed.
